For the purpose of this demonstration, assume each edit was made by a different user

The bottom of the four images below contains the original upload.

The second to bottom of the four images below contains some legitimate edit to the file, such as a crop or brightness adjustment. There can be any number of legitimate edits between the original and the usurp, including zero edits, it does not change how usurpation works.

The third from bottom of four images is the usurpation. It could be vandalism, but more often it's someone who just didn't look at what he or she was doing, or saw and didn't care.

The top image is a reversion to a previous version of the file. Note that as with the second from bottom image, there could be any number of additional edits after the usurpation, including zero. If there is any possible use for the original image, however, the usurpation should be reuploaded under another name and the original image should be restored.

Finally, for people that let their cameras name their images for them, or use very short names, my experience in working with files shows that the more common the file name, the more likely it is to get usurped.
