تریوزفسفات ایزومراز (انگلیسی: Triosephosphate isomerase) یک آنزیم است که در انسان توسط ژن «TPI1» کُدگذاری میشود. این ژن آنزیمی را میسازد که ۲ پروتئین کاملاً مشابه را شامل میشود و واکنش ایزومراسیون گلیسرآلدئید-۳-فسفات و دیهیدروکسیاستون فسفات را طی فرایندهای گلیکولیز و گلوکونئوژنز کاتالیز میکند.
اهمیت بالینی
در اثر جهش در ژن سازندهٔ این آنزیم، بیماری «کمبود تریوزفسفات ایزومراز» پدید میآید که مشخصهاش کمخونی همولیتیک، مشکلات حرکتی، استعداد فراوان در ابتلا به عفونتها و ضعف ماهیچهای (بهویژه ماهیچههای تنفسی و قلبی) است. کمخونی این بیماری از دوران نوزادی آغاز میشود که علتش همولیز (شکستن گلبولهای قرمز) است و باعث میشود تا شیرخوار دچار رنگپریدگی، خستگی شدید و تنگی نفس گردد. با شکستن گلبولهای قرمز، موادی چون آهن و بیلیروبین در خون بالا میرود و نوزاد، دچار زردی هم میگردد. مشکلات حرکتی از سن ۲ سالگی آغاز میشود که علتش اختلال در اعصاب حرکتی (موتور نرون) است که در نهایت منجر به آتروفی و تحلیل عضلانی، ضعف، لرزش و شلی ماهیچه میگردد. بیشتر مبتلایان بهسبب نارسایی تنفسی در سنین کودکی میمیرند و در موارد نادری از بیماری که آسیب نرونی شدید وجود ندارد، ممکن است بیمار تا دوران بزرگسالی زنده بماند.[۴]
